How do I access the NDIS?

If you meet the criteria and you would like to become a participant, you can either:

• Call NDIS on 1800 800 110 and ask to make an Access Request

• Complete and submit the Access Request Form and send by email to NAT@ndis.gov.au

• Contact your local LAC or NDIA office. Call NDIS on 1800 800 110 and ask to make an Access Request.

• Contact PROS (mobile number), if you meet the eligibility criteria and you require assistance to access the NDIS.

Do I have choices and control over my funded support?

You have choice and the control over how you use funded supports in your plan. That includes choice of how the supports are given and which service providers you use.

In some cases, the NDIA or others will manage the funding for supports. For example, where there is an unreasonable risk to a participant.

What types of supports are funded by the NDIS?

The types of supports that the NDIS may fund for participants include:

• Daily personal activities

• Transport to enable participation in community, social, economic and daily life activities

• Workplace help to allow a participant to successfully get or keep employment in the open or supported labour market

• Therapeutic supports including behaviour support

• Help with household tasks to allow the participant to maintain their home environment

• Help to a participant by skilled personnel in aids or equipment assessment, set up and training

• Home modification design and construction

• Mobility equipment

• Vehicle modifications

To access the NDIS, an individual;

1. Must be aged between 7 and 65

2. Must live in Australia and be an Australian Citizen or Permanent Resident or hold a Special Category Visa

3. Must require support from a person because of a permanent and significant disability and/or

5. May need some supports now to reduce future need
